10:10 Update: Master Plan for Aging (MPA) Update - Nhang Luong reviewed the state's Master Plan  
for Aging Forth MPA Annual Report."  
Five goals: 1) Housing for all ages and stages, 2) Health Reimagined, 3) Inclusion and Equity,  
Not Isolation, 4) Caregiving that Works, 5) Affording Aging  
Some key actions taken in 2024 to address “Housing for All Ages and Stages” goal:  
Expanded tax credits to develop more affordable senior housing.  
Expanded assisted living and other housing and care options under the Community Care  
Expansion (CCE) Program.  
Expanded affordable housing opportunities with the opening of an affordable housing apartment  
complex in Berkeley and funding allocation to California Native American Tribes to address  
housing and homelessness.  
Received federal funding from Centers for Medicare and Medicaid Services (CMS) to provide up  
to six months of transitional rent services to eligible Medical-Cal members who are experiencing  
homelessness or are at risk of homelessness, as part of the program, California’s BH-Connect  
(Behavioral Health Community-Based Organized Networks of Equitable Care and Treatment).  
Housing and Disability Advocacy Program (HDAP) operates in 56 counties and 17 tribal  
agencies since 2017, offers outreach, case management, benefits advocacy, and housing support  
to individuals who are likely eligible for disability benefits and who are experiencing or at risk of  
homelessness. In FY 2023-24 more participants (50%) exited permanent housing and the  
program received a $25 million annual ongoing appropriated under the Budget Act of 2024.  
The Home Safe Program operates in 58 counties and 23 trial agencies, supports the safety and  
housing stability of older adults and dependent adults, served by Adult Protective Services, who  
are experiencing homelessness or at risk of homelessness. In FY 2023-24, 61% of participants  
exited permanent housing. The Budget Act of 2024 reappropriated one-time funding, with funds  
set to expire June 30, 2026.

Marilyn Fowler presented to Concord Commission on Aging January 15, 2025. She is following  
up to schedule a presentation to the Concord City Council.  
Richmond Commission on Aging members will follow up to see if a presentation to the  
Richmond City Council can be scheduled.  
10:40 Update: Home Match Contra Costa - Logan Robertson, Director  
Ten rooms are soon to be available for rent. Most affordable room is $900, most expensive  
$1200. There are more rooms available than usual due to the program's expansion to West  
County and word getting out.  
10:45 Plan: 2025 Goals and Activities  
Possible topics for future meetings: Contra Costa Master Plan for Aging, presentations from state  
elected officials regarding the state budget, California Alliance of Retired Americans (CARA),  
tenant protections (cities of Concord and/or Antioch), update on eviction data, Measure X  
projects, and St. Paul's Commons apartments and community center.  
Each month, give 5 to 10 minutes to spotlight a member of the Housing Committee and agency.  
Gather more information about AB 474, a home match bill.
